As part of Phase 1 of a diocesan initiative to transition year-end reporting to a more user-friendly online format, two forms are now available for direct online completion (see “Submit Online” below).

- PDF Version – Print and complete manually.
- Excel Version (Recommended) – Complete electronically; calculations are automatic. Please print and sign before submitting.
